This is true if you\u2019ve changed your name to your spouse\u2019s name, hyphenated your name with your spouse\u2019s name, or changed your name to a previous name after a divorce or separation. If the name on your <a href=\"http:\/\/www.irs.gov\/pub\/irs-pdf\/fw2.pdf\" target=\"_blank\">Form W-2<\/a> doesn\u2019t match the name on file with the <span style=\"color: black;\"><a href=\"http:\/\/www.ssa.gov\/\" target=\"_blank\"><span style=\"color: #800080;\">SSA<\/span><\/a><\/span>, there may be a problem posting your earnings record to your social security account. Also, be sure not to include titles such as Dr. or Atty. and suffixes such as Jr. or Sr. with the name you provide to payroll or HR unless they are also listed on your social security card.<\/span><\/p>\n<p class=\"MsoNormal\" style=\"margin: 0in 0in 6pt;\"><span style=\"font-size: 10pt; font-family: Verdana;\">To change your name with <span style=\"color: black;\"><a href=\"http:\/\/www.ssa.gov\/\" target=\"_blank\"><span style=\"color: #800080;\">SSA<\/span><\/a><\/span>, file Form <a href=\"http:\/\/www.ssa.gov\/online\/ss-5.pdf\" target=\"_blank\">SS-5, Application for a Social Security Card<\/a>, with the <span style=\"color: black;\"><a href=\"http:\/\/www.ssa.gov\/\" target=\"_blank\"><span style=\"color: #800080;\">SSA<\/span><\/a><\/span>. This is also a good time to make sure your spouse files <a href=\"http:\/\/www.ssa.gov\/online\/ss-5.pdf\" target=\"_blank\">Form SS-5<\/a>, if necessary and also that any children who may have changed their name due to the marriage or divorce\u00a0do so as well.<\/span><\/p>\n<p class=\"MsoNormal\" style=\"margin: 0in 0in 0pt;\"><span style=\"font-size: 10pt; font-family: Verdana;\">Vision Payroll strongly recommends that <span style=\"color: black;\">employees periodically review their <a href=\"https:\/\/secure.ssa.gov\/acu\/ACU_KBA\/main.jsp?URL=\/apps8z\/ARPI\/main.jsp&amp;LVL=4\" target=\"_blank\"><span style=\"color: #800080;\">social security earnings record<\/span><\/a> and provide the <a href=\"http:\/\/www.ssa.gov\/\" target=\"_blank\"><span style=\"color: #800080;\">SSA<\/span><\/a> with the <a href=\"http:\/\/www.irs.gov\/pub\/irs-pdf\/fw2.pdf\" target=\"_blank\"><span style=\"color: #800080;\">Form W-2<\/span><\/a> to update any incorrectly posted earnings records.<\/span><\/span><\/p>\n","protected":false},"excerpt":{"rendered":"<p>If you got married or divorced and changed your name, be sure to give the changes to your payroll or HR department and to the Social Security Administration (SSA).
